One and Same person affidavit in Visa application

In my process of visa application, there's a minor name variation - "Gopal Singh Rathore M" in academic docs and "Gopal Singh Rathore" in others. which doesn't seem a big problem, but to be on the safer side. I've notarized a one and same-person affidavit. I have also come across a "One and Same Person Certificate," but it seems nonexistent. Is the notarized affidavit sufficient? also please clear up confusion regarding the One and Same Person Certificate. I'm from Tamil Nadu, PIN: 600091
