How can we transfer land ownership to my name or mother?

We have land along with a house in my village, where we have been living for over 80 years (my grandfather, father, mother, myself, and my baby). After my father's passing, we discovered that there are no documents for this land. We have been in possession of this land for more than 80 years and have been paying house tax, water tax, and electricity bills in my grandfather's name. How can we transfer the land ownership to either my name or my mother's name?
