How can I make proof of affidavit?

Hello, I have filed a complaint with the district consumer forum, and the Honorable Judge has requested me to submit a proof affidavit. Can someone help me with this, and kindly share if there is any template available for the same?

You can use a standard template for submitting a proof affidavit for your complaint in the district consumer forum. Start with your name, address, and a declaration stating that you are the complainant and are well aware of the facts of the case. The affidavit should be made to the effect that the information given in your complaint is true to the best of your knowledge and belief. The documents should present details about the complaint, like the nature of grievance and any supporting evidence. The affidavit must be signed in the presence of a notary public, as that is the only way it would be validated to be submitted in court. This eventually gives credence to your claim.
